Our case management team, comprised of an experienced RN and Social Worker, offers personalized, professional assistance to those in need of information and referral.  Our case management services include coordination of care for the elderly and physically disabled population who qualify for Medicaid, who would otherwise be placed in a nursing home Services paid for by this program may include Lifeline, Meals-On-Wheels, homemaking services, socialization opportunities, Galavan, durable medical equipment, assisted living residence.

The Home and Community Based Services (HCBS) Program allows people, who would otherwise be institutionalized to live in their own home, adult foster case home or assisted living facility.  People on the HCBS Program have the freedom to choose from a menu of services, which will best meet their needs.  There are two teams that work with the HCBS Program (see below).

The Case Management Team consists of a registered nurse and a social worker.  They work as a team to provide the client access to services that improve their quality of life and help maintain their independence.  Clients must be 65 years of age or older, Medicaid eligible, and meet nursing home level of care criteria.  If you qualify for and receive case management services you will receive help that allows you to stay in your home of choice.

The Gallatin Mental Health Center is a non-profit agency dedicated to meeting the mental health needs of the community.  At the present time we provide a variety of services - Adult Case Management, Outpatient Therapy, Crisis Response and Evaluations, and Psychiatric services.  We accept all insurances, Medicare, and Medicaid.  We are the designated contractor for the State's Mental Health Service Plan (MHSP) and we offer services on a sliding scale fee system, if necessary.

Support groups can be very helpful to people who find themselves in certain difficult situations.  Even people who think they can "handle it on their own" have gained immeasurably, so why not give it a try?  Call or check the Calendar in the Bozeman Daily Chronicle for meetings that may not be listed here.

LIVING WITH GRIEF SUPPORT GROUP
